Informations for PHSP NeoPlan Members


If you are considering opening NeoPlan PHSP account with us here is the procedure. First you should enquire about registration, sending us an email to info@neoplanlpus.com with the formal request and providing us with your email address which will serve as your online identification. We will probably ask you few questions establishing eligibility for PHSP and then register you in our system and open an online account for you.

You can also send us PHSP Registration request online, simply clicking on the Register link on the bottom of the page, and providing your contacts.

Then we'll send you credentials for your PHSP online account where you can login and download PHSP Application Form. Once you fill the form you can upload it to us and send us the registration fee along with the signed form (original required). Upon receiving the registration fee and the form we'll finish formal registration process and will notify you when your account is ready. For any questions related to the registration process you can send us an email or call us for clarifications.

As we collect sensitive information from you, please read and understand our security procedures and commitment to protect your privacy. You can find it on our web site under Privacy Policy link.

With the established online account it will be easier to you to exchange any PHSP related documents with us to speed up the process and turnaround time, but we still require originals from you (receipts, prescriptions) as per the Government policies.

Downloading the PHSP Application Form.

To download PHSP Application Form, login to your account and choose the link Submit/Download Forms in the Member Zone. On the next page [Forms Submission] click on link to Application Form, form will be opened for you and you can download this form and fill it in. Once you fill the form please sign it and send it to us along with the registration fee indicated in the form. You can also upload filled form to us for faster processing.

Uploading documents.

To upload document, login to your PHSP account and choose the link Upload files in the Member Zone. On the next page [Upload documents] choose the file to upload and click Upload button. Document will be uploaded to your Account. Please note, we can only process PDF and Windows Word documents for now, no Apple files please.

Submit the PHSP Claim.

To submit the PHSP Claim Form, login to your account and choose the link Submit/Download Forms in the Member Zone. On the next page [Forms Submission] click on link to the SUBMIT Claim Form (please be aware of two forms you can choose, regular Claim Form and Addendum Claim Form for any extended submissions that can't fit into the regular form), form will be opened for you and you can fill it in. Once form filled please print it, and then submit it clicking on Submit button on the Form. Printed form have to be signed and sent to NeoPlanPlus along with the check.
